What this site collects, where it goes, and how to reach us about it.
What the form collects
The “See it on your data” form asks for your name, email, organization, role and covered lives, and lets you add a message. Covered lives and the message are optional. The “Request access” form asks for your name, email, organization, role and what you need access to. That is everything the site asks for.
Where it goes
What you submit goes to Mailchimp, the email service we use. We use it to reply to you and to send the “See it on your data” email sequence. Every email carries an unsubscribe link.
What we do not do
- We do not sell what visitors send us.
- We do not share it with anyone else for their own use.
- We do not run third-party ad trackers on this site.
No patient information
This site does not accept protected health information. Please do not put patient details in the form. Work on real data happens later, under a signed agreement, in a sealed environment, never through this website.
Your requests
To see, correct or delete what you sent us, write to privacy@hyperspherehealth.com. You can also unsubscribe from any email we send.
Changes
If this page changes, the effective date at the top changes with it.
